Serious Case Review

Bristol Safeguarding Adults Board has published the report of a serious case review into the death of 18 year old woman with ADHD and autism who was murdered in 2014 by a 19 year old man. Both young adults were placed in an independent care home by their respective local authorities. Key issues identified include problems in the management of transition from children’s to adults’ services, lack of risk assessment and the use of out of area placements.
