The Health and Wellbeing Trainers are a forward thinking and dynamic team that works in South Sefton’s Virtual Ward, to improve health outcomes for adults with long term conditions. The successful applicant must be passionate about tackling health inequalities, and have a person-centered approach which encourages these adults to set achievable reablement goals.
Job Title: Health Trainer
Location: Sefton CVS, Burlington House, Waterloo
Hours: 35 hours per week
Accountable to: Reablement Co-ordinator
This is a rewarding role for anyone wishing to make a difference to the health and wellbeing of adults. You will attend weekly Multi-Disciplinary Team meetings and District Nurse Huddles to collect referrals, and maintain excellent working relationships with the existing leads such as GP’s, District Nurses, Community Matrons, Occupational Therapists and Physiotherapists.
The post will require you to conduct home visits as a lone worker. Experience in this field of work is desirable, along with excellent listening and communication skills. Your negotiation and leadership skills will enable you to deal confidently with clients and family members. You should be comfortable working both independently and as part of a team in order to manage this specialist area. Good planning and organisational skills are essential together with the ability to adapt quickly to meet the needs of different clients.
A willingness to travel throughout the South Sefton area in order to get the job done is important. This role will work in the Seaforth & Litherland area of South Sefton.
