Job Title: Hospital Discharge Service Support Worker
Grade: £20,750 per annum pro rata
Hours: 21 hours per week to be worked over 5 days
Contract: Fixed Term in the first instance – 31/06/2022

Accountable to: Hospital Discharge Service Coordinator

The Hospital Discharge Team are looking for a dynamic and forward thinking person to join this brand new service, to help Sefton residents settle back home following a stay in hospital, and to help prevent readmission. The successful applicant must be passionate about tackling health inequalities, and have a person-centered approach which encourages these adults to set up a supportive network of family, friends, and neighbours, as well as in their wider community.

This is a rewarding role for anyone wishing to make a difference to the health and wellbeing of adults. You will liaise with Southport and Aintree Hospital wards, as well as the hospital social work teams, checking referrals are appropriate, and made in a timely manner to ensure a suitable discharge service can be provided.

The post will require you to conduct home visits as a lone worker. Experience in this field of work is desirable, along with excellent listening and communication skills. Your negotiation and leadership skills will enable you to deal confidently with clients. You will also liaise with a team of Volunteers and need to ensure the discharges are evenly distributed between them based on their availability and locality. You should be comfortable working both independently and as part of a team in order to manage this specialist area. Good planning and organisational skills are essential together with the ability to adapt quickly to meet the needs of different clients and daily discharges.

A willingness to travel throughout the borough of Sefton in order to get the job done is important.

The post is initially funded until 30 June 2022.
