Job Title: Social Prescribing Care Coordinator
Salary: £30,160.65
Hours: 37.5 hours per week
Location: Southport Community Centre covering Southport and Formby Primary Care Network (PCN) area
Accountable to: Brighter Living Partnership/Senior Social Prescribing Link Worker

We’re looking for a motivated and compassionate Social Prescribing Care Coordinator to join our Social Prescribing Team. As the first point of contact you will play a vital role in managing referrals into the service from GP practices across Southport and Formby, ensuring that referrals are completed to a high standard and that all clients receive a triage telephone appointment to ensure suitability for the service. 

What you’ll do:

  • Review incoming referrals into the service from professionals to ensure they are completed to the required standard
  • Make initial triage phone calls to every client referred in, to introduce the service, complete the required triage admin questions and input on to the service systems.
  • Where immediate brief intervention referrals are identified as part of the triage process, direct clients to the most appropriate support (For example signposting to housing advice support or benefits advice drop ins), 
  • Work closely with senior staff to identify any referrals that may need reviewing or prioritising. 
  • Work closely with Social Prescribing Link Workers, GP practices and PCN teams
  • Input clinical data on EMIS and provide admin support for performance monitoring
  • Build strong relationships across community, health and voluntary sector services
  • Ensure safeguarding, confidentiality and personalised care standards are upheld

  • Highly organised with the ability to prioritise a busy workload
  • An excellent communicator with strong interpersonal skills
  • Confident working with diverse and sometimes vulnerable groups
  • Skilled in using IT systems (EMIS experience desirable)
  • Knowledgeable about safeguarding, personalised care and community services
  • A team player who can also work independently, with resilience and empathy
