The Social Prescribing service works with GP practices to identify and provide personalised support to individuals, their families and carers to take control of their wellbeing, live independently and improve their health and well-being.
Social Prescribing Link Workers support people in a person-centred way to improve their health and wellbeing, helping them to identify ‘what matters to them’ and develop an action plan to help them live healthier, happier lives by addressing non-medical issues. Part of this service involves connecting people to a wide range of community support and activities, such as making friends, sports clubs, learning opportunities, social activities and specialist support services and guidance, such as benefits advice or help to get back to work.
Social Prescribing Link Workers cover the following areas:
North Southport, Central Southport, Ainsdale & Birkdale, Formby, Crosby, Maghull, Seaforth & Litherland, Bootle.

Enhanced Care Social Prescribers
Enhanced Care at Home can help you get more out of life by supporting you with non-medical issues that may be affecting your health and wellbeing.
As part of our service we work with the Enhanced Health at Home Care Coordinators who will support you with the health elements of your support.
