The service helps adults to stay connected, informed and safe through interim support, community links and wellbeing activities.
We can connect people to a wide range of services and community activities such as:
- Befriending
- Wellbeing sessions
- Learning opportunities
- Meeting new people
- Volunteering
We can also link in with sources of specialist advice and guidance, such as support with benefits, housing or getting back into work.
Referrals must:
- Be awaiting adult social care assessment.
- Be discussed by a Social Worker at regular ‘Huddle’ meetings.
- Be able to access community support either independently or with the support of a carer.
- Have given consent to proceed.
We begin by meeting people in a way that feels right for them, exploring their goals and focusing on what matters to them most.
We’ll be here to provide support until a person leaves the adult social care waiting list, ensuring they have all the guidance, connections and resources they need.
