The High Intensity Use of Services team work one on one with patients who are frequent users of emergency services and non-elective admissions to A&E.

The role will focus on the co-ordination of the VCF sector voice through networks and in building cross sector partnerships to support the VCF sector’s role in local integrated care systems and the wider health system in relation to adults.

The Link Worker will work holistically to support individuals to take control of their health and wellbeing based on a ‘what matters to me’ approach by connecting people to a broad range of community groups and services.

The Community Connector will identify local champions and stimulate community resources to support individuals within the South Sefton locality.

The Sessional Worker will support the delivery of social activities for young people with additional needs and disabilities in accordance with the policies of Sefton CVS and at the request of the Buddy Up Coordinator.
