Job Purpose:

Sefton CVS in partnership with Cheshire & Merseyside ‘Health and Care Board’ are delivering a project working directly with Sefton residents who are regularly attending A&E, calling 999 and being admitted to hospital. This service has been expanded to include supporting those with serious mental illness, and those requiring support within a 48-hour response who will be referred by different pathways. 

Using a very human and 1:1 coaching approach, a team of ‘Outreach & Intervention Workers’ will work with this client group to find out any underlying social or emotional reasons they attend A&E as well as other services, and find ways forward. The ultimate outcome is for this client group to feel better supported and reduce health inequalities. The natural by-product of concentrating on people and what they need, is a reduction in the number of emergency ambulance calls and unscheduled care interactions. 

Our expectations of the ‘Outreach & Intervention Worker’ is that they will be a great listener and negotiator, highly self-aware with strong emotional agility, can connect with any individual despite their complexities and challenges in life; are open and can adapt to situations with a focus on supporting positive behaviour change.

This role follows a hybrid working model, with a team peer support day on Fridays.  Employees are expected to be onsite at Burlington House that day unless otherwise arranged. Hybrid working allows flexibility for remote work as well as the outreach element of the work, providing face to face support to clients on your caseload which could include home visits or support in the community.

A typical day as a HIU Worker involves connecting with clients through regular phone calls and visits, building trust and rapport to truly understand their needs. We support clients in engaging with other services and offer emotional support both over the phone and face to face.

Together with the client, we create a personalised reablement plan focused on building independence and confidence. We advocate for their needs during appointments and celebrate their progress with positive reinforcement. When the time is right, we work collaboratively to develop a closing plan, ensuring the client feels ready and supported to move forward on their own.

We have a positive working environment and have the support of our manager and peers to work in the best way possible, there is no manual or process to supporting people who come through the HIU service so using your own initiative and being able to assess clients needs and action plan collaboratively with the client is needed. Every day is different working on the HIU team, so the role is never mundane.
