The High Intensity Use of Services project is a dedicated team with a wide range of skills and experience, working one on one with patients who are stuck in a revolving door of high intensity use of emergency services and non-elective admissions to A&E.
Our Aim
– To reduce the number of inappropriate attendances to A&E and calls made to North West Ambulance Service (NWAS)
– To explore the underlying reason for frequent attendances to A&E and signpost and make referrals to the appropriate agencies that can support individual needs
– To reengage people with local community services
What we can do
– Using a 1:1 solution focussed approach, we can help to understand the underlying reasons for needing urgent care on a regular basis
– Liaise with relevant professionals and co-ordinate current package of care
– Signpost and make referrals to relevant agencies that can support individual needs
