The service is for people who receive very limited support from health or social care and no other informal support available from family or friends.
What does the Hospital Discharge Service provide?
The service aims to provide support to maintain or improve health and wellbeing, with rapid, timely support to manage concerns which may arise following discharge from hospital or similar health setting. By providing this support, the service in turn reduces the need for patients to be readmitted to hospital after discharge, by ensuring they have basic provisions in their property and that the accommodation is suitable to their needs.
The service also looks to maximise income through signposting and ensuring benefits and specialist debt management is available, as well as providing onward signposting and referral where needed.
