Sefton CVS and South Sefton and Southport and Formby CCG’s are piloting a new and exciting project to support high-intensity users of emergency NHS services.
We are seeking experienced, motivated individuals, with experience of working with people with complex needs as well as working within a fast-paced environment, to join our exciting new pilot project. The successful candidates will work with a rolling cohort of patients who attend A&E, call 999 or have been admitted to hospital more than might be expected. We are looking for individuals whose ability to connect with patients is strong enough to be able to change the behaviours of people with even the most complex medical/social needs. The post holders will seek to appropriately reduce the number of emergency ambulance calls and unscheduled care interactions, emergency contacts and other care interactions across South Sefton and Southport and Formby CCG areas.
Reporting to the High Intensity User Co-ordinator, the Outreach and Intervention Workers will work with people to focus on positive behaviour change addressing those issues that are contributing to inappropriate care seeking behaviours.
This role is fixed term for 12 months in the first instance and is subject to an enhanced DBS check.
