The Network Development Officer will provide co-ordination and administrative support to the Forum Co-ordinators in bringing together the VCF sector voice through networks and cross sector partnerships.

The Social Prescribing Link Workers (SPLWs) work holistically to support individuals to take control of their health and wellbeing based on a ‘what matters to me’ approach by connecting people to a broad range of community groups and services.

You will play a vital role in managing referrals into the service from GP practices across Southport and Formby, ensuring that referrals are completed to a high standard and that all clients receive a triage telephone appointment to ensure suitability for the service.
