Social Prescribing Link Worker (Full-Time)
Seaforth & Litherland (closing 12noon on Monday 25th January 2021) 

Job title:Social Prescribing Link Worker
Salary: £25,712.92
Location:  Seaforth and Litherland PCN
Hours: Full-time post at 35 hours per week
Term: until end Dec. 2021 with possible extension
Accountable to: Sefton CVS Line Manager

The Social Prescribing Link Worker will be employed by Sefton CVS and will work as part of one team, with 10 other Link Workers across 2 host organisations, providing a social prescribing service to GP practices, aligned to the Living Well Sefton model.

The Link Worker will work holistically to support individuals to take control of their health and wellbeing based on a ‘what matters to me’ approach by connecting people to a broad range of community groups and services. The Link Worker will support people to increase their resilience and reduce the impact of health inequalities by addressing the wider determinants of health such as debt, poor housing etc.

The Link Worker will also support existing VCF sector groups to be sustainable; identify gaps in services, and work with the Sefton CVS Group Development Team to develop new initiatives to meet local needs.

Hours: Part-Time post at 17.5 hours per week
Term: until end Dec. 2021 with possible extension
Accountable to: Sefton CVS Line Manager

Brighter Living Partnership Project Worker – Shopping Service

Job Title: Project Worker – Shopping Service
Grade: £18,562 (pro rata = £14,850)
Hours: 28 hours per week (6 month contract initially but possibility of extension)
Location: Brighter Living Partnership, Southport Community Centre, Norwood Road or home working
Accountable to: Deputy Manager

Job Purpose:

  1. To run the day to day operation of the shopping service including managing volunteers and directly liaising with clients as required
  2. To work with other Brighter Living staff and external partners to ensure clients are receiving the support they need in all aspects of health and wellbeing
  3. Explore development opportunities to make the project more efficient, sustainable and address any emerging needs
