Community Cancer Navigators (closing 12noon, Wednesday 18th November 2020) 

Salary: £23,398 full-time or pro rata
Location: Southport (based with the Southport and Formby Cancer Information and Support Centre or Waterloo based with Sefton CVS)
Hours: 35 Hours (Full Time)./ Part time
Contract: Currently funded till 30th May 2021
Accountable to: Project Lead

Sefton CVS are working in partnership with Southport Cancer information and Support Centre  and the Sefton Clinical Commissioning Groups deliver the Community Cancer Navigator Service in Sefton. Delivering on Macmillan’s commitment to ensure that no one should face cancer alone we are now inviting people to apply for Community Cancer Navigator positions which are  either full time (35 hours) and part-time (hours negotiable). These posts are currently funded until 30th May 2021.

Working with a limited number of cancer patients the Community Cancer Navigators work in partnership with local GPs, Hospital Lead Cancer Nurses and Clinical Nurse Specialists, to offer an electronic holistic needs assessment (eHNA) to patients diagnosed with cancer, and where identified, agree a personalised care plan with the patient to support them to meet their identified concerns and achieve their goals.  The Community Cancer Navigator will be a practical resource to help connect individuals into services at a local level to aid independent self-management of their treatment, and improve their health and wellbeing.

If you are passionate about ensuring people affected by cancer receive the best possible care and have the resilient qualities needed to guide and support patients through and beyond treatment then this post may be for you.  The successful candidates will have knowledge of cancer care as a whole, and more specifically, the Recovery Package, and will have a proven track record of supporting people in the community to improve their health and wellbeing.
