Sefton 4 Good
Sefton 4 Good is a charity initiative from Sefton Council for Voluntary Service (CVS). It encourages local people to come together and give their time, skills, money and resources to help local good causes.
Health & Wellbeing Trainers
The Health & Wellbeing Trainers are part of the Integrated Care Team in South Sefton, which also includes Community Matron, District Nurses, Social Workers, Mental Health Team, Occupational Therapy, Medicines Management, Physiotherapist and Community Dietician.
Hospital Discharge Service
The Sefton CVS Hospital Discharge Service aims to provide free, practical support for Sefton residents, for up to six weeks after discharge from a hospital or health setting.

High Intensity Use of Services Team
The High Intensity Use of Services team are dedicated to working one on one with patients who are frequent users of emergency services and non-elective admissions to A&E.

Sefton’s Crisis Cafe’s offer out of hours drop-in support Friday, Saturday and Sunday, 5pm -11pm to anyone experiencing a mental health crisis, giving adults aged 18+ a place of safety as an alternative to A&E.
Social Prescribing Link Workers
Social Prescribing Link Workers support people to improve their health and wellbeing, helping them to identify ‘what matters to them’ and develop an action plan to help them live healthier, happier lives by addressing non-medical issues.
Community Connectors
The Community Connectors work with anyone over the age of 18 to help them get out and about more in their local community by going along to groups or social activities and meeting new people.
