Living Well Sefton
Living Well Sefton is a free service with a focus on supporting people with issues that may be affecting their health and wellbeing. Learn more

The Merseyside Offender Mentoring Project (MOMS) offers a range of support to prisoners pre and post release, from practical help to a listening ear. Learn more

Health & Well Being Projects

The Health & Wellbeing Trainers are part of the Integrated Care Team in South Sefton, which also includes Community Matron, District Nurses, Social Workers, Mental Health Team, Occupational Therapy, Medicines Management, Physiotherapist and Community Dietician. Learn more
The Sefton CVS Hospital Discharge Service aims to provide free, practical support for Sefton residents, for up to six weeks after discharge from a hospital or health setting. Learn more
The High Intensity Use of Services team are dedicated to working one on one with patients who are frequent users of emergency services and non-elective admissions to A&E. Learn more
Sefton's Crisis Cafe's offer out of hours drop-in support Friday, Saturday and Sunday, 5pm -11pm to anyone experiencing a mental health crisis, giving adults aged 18+ a place of safety as an alternative to A&E. Learn more
Social Prescribing Link Workers support people to improve their health and wellbeing, helping them to identify ‘what matters to them’ and develop an action plan to help them live healthier, happier lives by addressing non-medical issues. Learn more
The Community Connectors work with anyone over the age of 18 to help them get out and about more in their local community by going along to groups or social activities and meeting new people. Learn more

The Diverse Ethnic Backgrounds (DEBs) Community Development Service is dedicated to achieving inclusion and improving the quality of life of all of Sefton’s diverse communities. Learn more

Sefton Young Advisors are professionally trained to provide links between professionals, young people and their communities, ensuring that plans and delivery are relevant and respected. Learn more
SEAS is a group of local charities who have come together with the aim of helping children, young people and families to aspire and achieve through providing bespoke emotional well-being support. Learn more
Buddy Up is a mentoring and befriending project for young people aged between 13 and 18 who have additional needs and disabilities and are at risk of social isolation. Learn more
Sefton 4 Good, Heritage and Digital Inclusion
